APPLICATION FORM: Complete the entire application and provide digital signature APPLICATION FEE: The NON-REFUNDABLE application fee MUST be paid before application is considered. STANDARDIZED TEST RESULT: Copies of standardized test results (CTBS, ST, CAT, IOWA, ETC.) are requested if available for grades 1-6. Obtain a copy of the MOST RECENT testing results from the present school and submit to St. Paul’s Lutheran School office. If no test results are available, please let us know. BIRTH CERTIFICATE / SOCIAL SECURITY CARD: All applications MUST be accompanied with a copy of the child's birth certificate and a copy of social security card. IMMUNIZATION FORMS: Original immunization forms are required by the state and MUST accompany the application form. Applications cannot be considered until all requested forms have been submitted. St. Paul's admits students of any race, color, national and ethnic origin to all programs and activities. KINDERGARTEN Provide the school with copies of any readiness testing results available. A readiness test will be given to any student that is new to St. Paul’s. The readiness test will be scheduled with the K teacher prior to the beginning of the school year. FEES / TUITION FOR K-8 The fees listed below are due at the time of enrollment to secure a place in the class: K - 8 Application Fee - $100 (ALL NEW STUDENTS) K - 8 Enrollment Fee - $525 (if paid before April 26) / $575 (if paid after April 26) DUE BY AUGUST 10 Tuition for August * Tuition is $255. Per month for SPLC and Affiliate LCMS Church Members. Tuition is $510 per month for non-members. DISCOUNT 10% off annual tuition if paid in full by April 23, or 5% off annual tuition if paid in full by July 2.
